Permanent Jewelry Consent

I hereby consent to the procedure of receiving permanent jewelry. I understand and acknowledge the following:

Purpose

The application of permanent jewelry is a non-invasive procedure that includes welding jewelry together in place of a clasp. The procedure will be performed by a trained and experienced practitioner.

Risks and Complications

I am aware that there are potential risks and complications associated with permanent jewelry placement, including but not limited to infection, allergic reactions, discomfort or irritation, burns, scarring, and the need for further medical intervention or removal.

Aftercare

I will adhere to the aftercare instructions provided by the jewelry practitioner to ensure the proper maintenance of the permanent jewelry.

Maintenance

I acknowledge that proper maintenance, including regular cleaning and tightening of the jewelry, may be required. Neglecting maintenance may decrease the lifespan of the jewelry.

Personal Responsibility

I take full responsibility for the jewelry after it is placed, including any potential damage, loss, or theft. I acknowledge that, although this jewelry is intended to be permanent, durable, and long-lasting, it is not indestructible and the length of wear can vary depending on physical activity and other factors. I understand that the studio or technician will not be liable for any such incidents.

Consent to Procedure

I consent to the permanent jewelry placement procedure and acknowledge that the practitioner has explained the procedure, risks, and aftercare instructions to me. I have had the opportunity to ask questions and have received satisfactory answers.
